Why Did Evolution “Allow” Heart Disease?

Here’s a fascinating thought: maybe heart disease isn’t simply a design flaw. Maybe it’s part of an evolutionary trade-off.

Evolution doesn’t have one goal—to help us live to 100. Its primary goal is to help us survive long enough to reproduce and raise the next generation.

When we’re young, our bodies are flooded with hormones and biological systems that promote growth, energy, fertility, and healing. Many of those same systems also encourage storing energy, building tissues, and maintaining enough cholesterol to make hormones and support reproduction. Those traits were incredibly valuable when food was scarce and life was physically demanding.

The catch is that the same biology that helps us succeed at 20 or 30 can contribute to problems at 70 or 80. Over decades, cholesterol can accumulate in artery walls, inflammation can take its toll, and blood vessels gradually stiffen.

From evolution’s perspective, that may not have mattered very much. For most of human history, relatively few people lived into their 80s or 90s. Natural selection strongly favored traits that improved reproductive success early in life, even if those same traits had negative effects many decades later.

So, in a sense, evolution made a trade-off: optimize the body for surviving, reproducing, and raising children—not necessarily for reaching 95 with perfectly clean coronary arteries.

The good news is that we don’t have to accept that trade-off. Modern medicine, better nutrition, regular exercise, and healthier lifestyles allow us to work against some of those evolutionary compromises. Understanding where we came from helps explain why heart disease exists—but it also reminds us that today we have opportunities our ancestors never had.
